How do I retrieve my passport application?

Download e-Form. Applicants using eForm, download soft copy of the eForm, fill the same off-line and then must upload it back. The printed copy of the eForm will not be accepted at PSK/RPO.

How do I find my passport file number?

You will be given a passport reference file number when you apply for a passport and submit the application form. You will need to pay a registration fee when submitting the application form, following which you will be given a passport reference file number.

What happens if you say you lost your passport?

To replace a lost or stolen passport, you must submit Form DS-11 in person. You may be eligible to make an appointment to apply in person at a passport agency or center to replace your passport.
